Asthma Action Plan

The Asthma Action Plan is a personalized self-management plan with graphics and simple language on a triplicate NCR form prescribed for patients/parents with asthma. It instructs them on how to recognize the presence of asthma warning signs, their severity levels and what actions to take during asthma episodes. The National Heart Lung & Blood Institute’s (NHLBI) Expert Panel Report 3: Guidelines for the Diagnosis & Management of Asthma 2007 strongly recommend the use of written action plans to increase self-management skills.
