Disease Management / PECS

QueensCare Family Clinics is just one of the many community clinics that utilize a disease management program to educate and empower patients with chronic diseases. This program is based on the Chronic Care Model (CCM) espoused by L.A. County Department of Health Services and the Community Clinics Association of L.A. County. Patient data for key measures are entered into the Patient Electronic Care System (PECS). PECS is a database for tracking and reporting quality of care, used by clinics involved in health collaboratives of the Health Disparities Network, such as the California Primary Care Association.

Chronic diseases such as diabetes, hypertension and asthma can be influenced by lifestyle behaviors such as a healthier diet and increased physical activity. The Disease Management team of providers, case management staff and clinical pharmacists lend that extra support to patients in the care of their chronic disease. Case managers are present in all the clinics to provide classes and one-on-one patient counseling.

 

 

Pediatric Asthma Disease Management Program (PADM)

The Pediatric Asthma Disease Management Program (PADM) at QueensCare Family Clinics offers a state-of-the-art, integrated model of pediatric asthma prevention and treatment. Its purpose is to measurably improve the diagnosis, treatment and control of asthma in QFC’s pediatric patients. PADM is based on the Bureau of Primary Health Care’s Chronic Care Disease Management Model and incorporates national guidelines and “best practices” into its operational modalities.

e.n.e.r.g.y.

e. n. e. r. g. y. (eating nutritiously, exercising regularly & growing “y”-isely ) is a pediatric weight management program inaugurated in October 2005 by QueensCare Family Clinics (QFC). The clinic-based program is a family centered model that’s interactive, upbeat, and fun for everyone. Dr. Jason Lue provides oversight and direction as e. n. e. r. g. y. ’s Program Director and physician champion; and Maria Dorado, RN serves as the fulltime Project Director. A team of highly-trained community health workers provide a family-centered, culturally-sensitive, and age-appropriate approach to the prevention of childhood obesity in 6 to 17 year olds. e. n. e. r. g. y . supports and educates not only the target child, but also the whole family.
