Welcome New Patients

Thank you for choosing QueensCare Family Clinics for your medical care. We offer a wide range of clinical services with a comprehensive and preventive approach for your care. Our staff would like to make your experience with our office a pleasurable one. We pledge to treat our patients and your families with courtesy, dignity, respect and professionalism.

Information for your first clinic visit:

  • For your appointment, you will be given a designated arrival time to complete the registration process and any necessary paperwork required by your insurance plan or QueensCare Family Clinics.
  • Please bring your insurance card and photo identification to every visit. We accept most major insurance plans including: Medi-Cal, Medicare, Healthy Way L.A., Healthy Families and Healthy Kids.
  • Please dress comfortably. While not required, we recommend you consider loose fitting clothing that is easy to remove or easy to expose the area that you are concerned about since the doctor may need to examine those areas.
  • Please bring all your medications with you to every visit.

General Payment Policies

QueensCare Family Clinics will bill your insurance provider for Professional Service Fees provided during the course of your visit. It is our policy to collect all co-pays, deductibles and fees not covered by your insurance at the time of visit. If you do not have insurance, our Certified Application Assistants will help enroll you in a health plan that best meets your family’s needs. We also offer a sliding fee scale for those who do not qualify for any program and have no private insurance. Full payment is expected when services are provided. We accept cash, MasterCard, Visa and Discover cards as means of payment.

If you have questions regarding your appointment, or our office policies, please call us at (800) 454 – 1800. Our Call Center is available Monday through Friday from 7:00 am until 6:30 pm to schedule appointments and answer any questions you may have.
